Existing Patients

If you are an existing patient and your patient portal account has been enabled by our office, please click here to log in using your username and password.

You may also use your your cell phone for verification by text.

If you do not have your username and password, please have your account enabled next time you are in the office. You will need an email address.

HEALOW APP for Existing patients

As an existing patient you can access your patient portal on a smart phone. Download the Healow App from the App Store or Google Play and use our Practice Code CCFDBD, then log in using your username and password. 

New Patients

If you are a New Patient, click here pre-register to reduce your time filling out some paperwork in the offce.
